Exercise does more than build muscle—it may directly protect the brain. A recent study from Korea University Guro Hospital found that irisin, a protein secreted by muscles during exercise, is closely linked to the accumulation of beta-amyloid in the brain, a core pathology of Alzheimer's disease.

Key Finding: Lower Blood Irisin Tied to Higher Brain Amyloid

The research team conducted blood tests, amyloid PET scans, and neuropsychological assessments on 100 participants. Subjects were divided into four groups: amyloid-negative with normal cognition, amyloid-positive asymptomatic, amyloid-positive with mild cognitive impairment, and Alzheimer's dementia.

Results showed:

  • All three groups with confirmed brain amyloid accumulation had significantly lower blood irisin concentrations compared to the amyloid-negative, cognitively normal group.
  • Blood irisin levels were already significantly reduced at the early stage when amyloid began accumulating, even while cognitive function remained normal.
  • Higher blood irisin concentrations were associated with lower centiloid values, indicating less amyloid buildup in the brain.

Irisin: A Key Biological Link Between Muscle, Brain, and Cognition

Through mediation effect analysis, the team confirmed that decreased irisin is linked to increased brain amyloid accumulation, which in turn may lead to cognitive decline. This demonstrates that irisin is an important biological link connecting the core pathology of Alzheimer's disease with cognitive impairment.

Professor Kang Sung-hoon of the Department of Neurology at Korea University Guro Hospital stated: "The significance of this study is that we confirmed in actual humans that irisin, which increases through exercise, is closely connected not only to muscle health but also to the core pathology of Alzheimer's disease. We expect this will serve as important evidence for developing blood-based biomarkers using irisin, as well as for establishing dementia prevention strategies through exercise and improved muscle function."

The findings were published in the journal Alzheimer's & Dementia: Diagnosis, Assessment & Disease Monitoring.
