The study, published in the journal 'Alzheimer's & Dementia', analyzed brain scans and cognitive assessments from 459 adults aged 60 and older in India. It found that gray matter loss remains the strongest predictor of cognitive decline, but the health of white matter, especially superficial white matter, also plays a critical role.

When the integrity of superficial white matter was poorer, the link between gray matter loss and worse language performance and cognitive impairment was stronger. Conversely, when superficial white matter was healthier, these associations were weaker. This suggests that healthy white matter 'wiring' may provide a form of 'resilience', helping to buffer the impact of gray matter loss.

Superficial White Matter: A Potential Source of Cognitive Resilience

First author Yingxu Liu, a postdoctoral scholar at the University of Southern California's school of medicine, stated, 'Cognitive health depends not only on how much gray matter is preserved, but also on the condition of the wiring that connects it.' The study also found that healthier superficial white matter was most consistently associated with stronger language skills, such as word recognition and fluent speech.

Future Directions: Preserving Brain Connections

This is among the first studies to examine the brain's superficial white matter in a community-based population from a low- and middle-income country. Senior author Leon Aksman noted, 'Two people with a similar degree of gray matter loss may not experience the same cognitive effects if the local connections surrounding that gray matter differ in health.' Future longitudinal studies are essential to test whether preserving these connections can help maintain cognition.
